Job summary

UnitedHealth Group is seeking a Care Coordinator to manage a panel of members with chronic and complex health care needs. The role supports clinical and non-clinical activities to promote person-centered care coordination.

Location flexibility includes Caldwell, ID with remote options. The role requires a Idaho license and experience with Medicaid/Medicare and care coordination in a team-based setting.

Qualifications

• Care coordination experience in healthcare settings.
• MS Office (Word, Excel, Outlook) proficiency.
• Ability to travel within assigned territory up to 50%.
• HIPAA/PHI handling and data security awareness.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Engage members face-to-face and/or telephonically for needs assessments.
• Collaborate with internal care team and community resources to implement care plans.
• Assist members with HCBS supports and services.
• Provide referrals to internal or community resources.
• Support discharge planning and care transitions after ER or SNF.
• Educate and coach for self-management of care needs.
• Advocate for member needs with the health care team.
