Qualifications

• Must hold an active Idaho license in one of the listed healthcare fields (LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT, or LPN/LVN) or have 2-year degree with 2+ years healthcare experience.
• 1+ years of experience with Medicaid/Medicare or significant SDoH needs.
• 1+ years of MS Office (Word, Excel, Outlook) experience.
• Access to a designated quiet workspace and ability to secure PHI.
• Residence in an area with reliable internet and the ability to travel up to 50%.
• Reside in Idaho or be able to work in Idaho.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Engage members in comprehensive needs/wellness assessments and develop person-centered care plans.
• Collaborate with internal care teams, providers, and community resources to implement plans and remove obstacles.
• Assist members with obtaining HCBS supports and services.
• Provide referrals and linkage to internal or community resources.
• Support discharge planning and care transitions after ER/inpatient/SNF.
• Educate and coach members for self-management and health lifestyle changes.
• Advocate for members to ensure their needs are represented by the health care team.
• Coordinate nonclinical provider questions with Health Plan and UHC resources.
