Job summary

UnitedHealthcare is seeking a Care Coordinator to manage a panel of members with chronic and complex health needs. You will conduct comprehensive assessments, develop person-centered care plans, and collaborate with internal teams to remove barriers to care.

Qualifications

• Must meet one of the following licenses or a 2-year degree with 2+ years healthcare experience
• Experience with Medicaid/Medicare or strong social determinants of health (SDoH) background preferred
• Proficiency with MS Office (Word, Excel, Outlook) and documenting in EHRs

Responsibilities

• Manage a panel of members with chronic and complex health care needs
• Develop individualized, person-centered care plans with goals and interventions
• Coordinate with providers and community resources to remove barriers and support community living
• Assist members with HCBS supports and services and provide referrals as appropriate
• Support discharge planning and care transitions after ER visits or facility stays
• Educate and coach members for self-management of health and lifestyle changes
• Advocate for patient needs within the health care team and with external providers
