Job summary

UnitedHealth Group's UnitedHealthcare division is seeking a Field Care Coordinator in the Santa Fe, NM area. This full-time, 40-hour week role is field-based with a home office and occasional overtime; remote work is possible if aligned with business needs.

You will assess member needs, develop person-centered care plans, coordinate with providers, and support transitions of care to improve quality and utilization.

Qualifications

• Bachelor's degree OR 2+ years of health care experience.
• 1+ year of experience with MS Office (Word, Excel, Outlook).
• Driver's license and ability to travel up to 50% of the time (NM territory).
• Dedicated home workspace with reliable internet.

Responsibilities

• Engage members face-to-face and/or by phone to complete comprehensive needs assessments.
• Develop and implement person-centered care plans addressing medical, behavioral, and social needs.
• Coordinate with internal care teams, providers, and community resources to execute care plans.
• Provide education and coaching to support member self-management.
• Support discharge planning and care transitions after ER visits or SNF stays.
• Advocate for members and families to ensure needs are met by the care team.
• Build positive relationships with members and maintain cultural sensitivity.

Education
