Job summary

UnitedHealthcare in Caldwell, ID is seeking a Field Care Coordinator to act as the primary care manager for a panel of members with chronic and complex health needs. You will coordinate medical, behavioral, and social drivers of health to promote appropriate service use and improve quality of care.

This remote-friendly role supports a person-centered care approach, with collaboration across care teams, providers, and community resources.

Qualifications

• Current Idaho license in one of the specified social work or nursing roles, or RN license.
• 1+ year experience with Medicaid/Medicare or SDoH needs.
• 1+ year of MS Office experience.
• Quiet home workspace secured for PHI.
• Reliable high-speed internet in home.
• Ability to travel within territory up to 50%.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs and wellness assessments with medical, behavioral, functional, cultural, and social drivers of health.
• Collaborate with care team, providers, and community resources to implement care plans.
• Assist members with HCBS supports and services access.
• Provide referrals to internal or community resources as appropriate.
• Support discharge planning and care transitions after ER visits or SNF stays.
• Educate and coach members for self-management of care needs and lifestyle changes.
• Advocate for members to ensure needs and choices are represented.

Education

Idaho license (LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT) or RN
