Job summary

UnitedHealthcare is seeking a Field Care Coordinator to serve members in the Santa Fe, NM area. This field-based, remote-friendly role focuses on comprehensive care management for members with medical and behavioral needs, promoting appropriate service use and quality outcomes.

You will operate from a home-based office, travel up to 50% across NM, and collaborate with care teams to develop person-centered plans.

Qualifications

• Bachelor’s degree OR 2+ years of health care experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Driver's license and travel up to 50% of the time to NM territory.
• Designated home workspace with high-speed internet.
• Currently reside in Santa Fe, NM area.

Responsibilities

• Engage members face-to-face and/or telephonically to complete needs assessments and coordinate care.
• Develop person-centered care plans addressing medical, behavioral, and social determinants of health.
• Collaborate with internal care teams, providers and community resources to implement plans.
• Provide education and coaching to support self-management and healthy lifestyle changes.
• Coordinate discharge planning and care transitions following ER visits or SNF stays.
• Advocate for members to ensure needs and choices are respected by the care team.
• Create positive member experiences and build trust with the community.
• Learn cultural contexts and tailor care to diverse populations.
