Job summary

UnitedHealth Group is seeking a Field Care Coordinator in Roswell, NM. This field-based role manages a panel of members with low-to-medium complexity medical/behavioral needs, coordinating care and services to improve outcomes.

The position is full-time (40 hours/week) with flexibility to work 8-hour shifts within 8am–5pm, and telecommuting is possible from home when not in the field. Candidates must have a healthcare background, licensure if applicable, and be within commuting distance of

Qualifications

• Bachelor's degree or 2+ years of health care experience.
• Current, unrestricted licensure in RN or LCSW/LMSW/LMHC with 3 years clinical experience.
• 3+ years of clinical experience in health care.
• 4+ years in a clinical environment including 1+ year as Care Coordinator.
• LPN with 3+ years clinical experience.
• 1+ years MS Office experience.
• Driver's license and ability to travel up to 50% within territory.

Responsibilities

• Engage members face-to-face or by phone for needs assessments.
• Develop and implement person-centered care plans.
• Collaborate with care team and community resources to execute plans.
• Provide education to support self-management of care needs.
• Coordinate discharge planning and care transitions following ER or SNF events.
• Advocate for members to ensure needs are represented by the health care team.
• Establish a positive relationship with members and ensure cultural competence.
