UnitedHealth Group is seeking a Field Care Coordinator for remote work in New Mexico. You will manage a panel of members with low-to-medium medical/behavioral needs, coordinating care across providers and community resources to improve outcomes.

The role requires a bachelor's degree or 2+ years healthcare experience, MS Office proficiency, and ability to travel up to 50% within NM. Home-based office and flexible scheduling are provided.

• MS Office proficiency (Word, Excel, Outlook).
• Driver's license with travel up to 50% to meet members/providers.
• Home workspace with reliable high-speed internet.

• Engage members face-to-face or by phone to assess medical, behavioral and socioeconomic needs.
• Develop and implement person-centered care plans for chronic conditions and wellness.
• Collaborate with care team, providers and community resources to execute plans.
• Educate and coach members for self-management and lifestyle changes.
• Support discharge planning and care transitions following ER or SNF.
• Advocate for member needs and ensure their preferences are represented.
• Foster a positive member experience and culturally sensitive care.
