UnitedHealth Group is seeking a Field Care Coordinator to serve a panel of members with medical and behavioral needs. You will assess, plan, and coordinate care across medical, behavioral, and social determinants to promote appropriate service use and improved outcomes.

This 40-hour-per-week, field-based role offers a home-based office. Travel up to 50% within the assigned territory, with strong collaboration with care teams and community resources to deliver culturally competent care.

Competenze

• Bachelor's degree or 2+ years of relevant health care experience required.
• Licensed Clinical Social Worker (LCSW), LMSW, LMHC or equivalent with 3+ years clinical experience preferred.
• 4+ years of experience in a clinical environment (including 1+ years as a Care Coordinator) and/or LPN with 3+ years clinical experience.

Mansioni

• Engage members to complete comprehensive needs assessment across medical, behavioral, and social needs.
• Develop person-centered care plans addressing chronic conditions, wellness, and social determinants.
• Coordinate with care team, providers, and community resources to implement care plans.
• Provide education and coaching to support self-management and healthy lifestyle changes.
• Support discharge planning and care transitions after ER visits or SNF encounters.
• Advocate for member preferences and ensure their needs are represented by the care team.
• Cultivate positive member experiences and culturally sensitive care.
• Learn community cultures and conduct comprehensive needs assessments (CNA).
