Job summary

UnitedHealth Group in Caldwell, ID is seeking a Field Care Coordinator to serve as the primary care manager for a panel of members with chronic and complex health needs. This remote role supports a person-centered care approach, coordinating medical, behavioral, and social needs to improve quality of care and utilization.

You will collaborate with internal teams, conduct needs assessments, develop individualized care plans, and arrange HCBS supports, with travel up to 50% as required.

Qualifications

• Current clinical license in Idaho or RN license.
• Experience with Medicaid/Medicare or social drivers of health (SDoH).
• Proficient with MS Office (Word, Excel, Outlook).
• Ability to maintain PHI securely and work from a designated home workspace.

Responsibilities

• Serve as primary care manager for dual-eligible members.
• Conduct comprehensive needs/wellness assessments and develop person-centered care plans.
• Collaborate with care teams, providers, and community resources to execute plans.
• Assist with HCBS supports and service referrals.
• Coordinate care transitions after ER/inpatient/SNF events.
• Educate and coach members for self-management of care needs.
• Advocate for member needs within the health care team.
