Job summary

UnitedHealthcare is seeking a Care Coordinator to be the primary care manager for a panel of dual eligible members, focusing on medical, behavioral and social needs to improve quality of care. The role emphasizes person-centered care and collaboration with the care team to address barriers and coordinate services.

The position supports flexible remote work from Idaho Falls or surrounding areas, with travel as needed to meet members and providers, and opportunities to engage with community

Qualifications

• Current LCSW, LMSW, LSW, LCPC, LPC, LMFT, or LAMFT in Idaho.
• 1+ years of Medicaid/Medicare or SDoH-related experience.
• 1+ years of MS Office (Word, Excel, Outlook) experience.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs and wellness assessments addressing medical, behavioral, functional, cultural and social drivers of health.
• Collaborate with internal care teams, providers and community resources to implement care plans and remove obstacles for community integration.
• Assist members with HCBS supports and services.
• Provide referrals and linkages to appropriate internal or community resources.
• Support proactive discharge planning and care transitions after ER visits, hospitalizations, or SNF stays.
• Educate and coach members for self-management and healthy lifestyle changes.
• Advocate for members to ensure their needs are represented by the health care team.
• Assist with nonclinical questions for providers and facilities and connect them to the right Health Plan resources.
