UnitedHealth Group is seeking a Field Care Coordinator in Roswell, NM to manage a panel of members with low-to-medium complexity medical/behavioral needs. This field-based role operates from a home-based office, with 40 hours/week and occasional overtime to support member care and transitions.

The position requires a diverse set of qualifications from clinical licensure to 4+ years in clinical settings, and travel up to 50% within the territory to meet members and providers.

Qualifications

• Must meet one of the listed degree/experience paths.
• Clinical licensing requirements where applicable.
• Experience in care coordination and health systems.

Responsabilités

• Engage members in needs assessment across medical, behavioral, and socioeconomic domains.
• Develop and implement person-centered care plans for chronic conditions and wellness.
• Collaborate with care team to execute care plans and coordinate transitions.
• Provide education to support self-management and health literacy.
• Support discharge planning and care transitions after ER/hospital/SNF stays.
• Advocate for member preferences within the care team.

LPN with 3+ years clinical experience
