Qualifications

• Current and unrestricted Idaho license in one of the listed professional licenses.
• 1+ years of experience with Medicaid/Medicare or SDoH.
• MS Office (Word, Excel, Outlook) proficiency.
• Driver’s license and ability to travel up to 50% within territory.
• Quiet home workspace with PHI security and reliable internet.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs/wellness assessments and develop person-centered care plans.
• Collaborate with internal care team, providers, and community resources to implement plans.
• Assist members with HCBS supports and services; provide referrals as needed.
• Coordinate care transitions following ER visits or SNF stays.
• Educate and coach members for self-management and healthy lifestyle changes.
• Advocate for members to ensure their needs are represented by the care team.
• Address provider and facility nonclinical questions, connecting to Health Plan resources.

Idaho licensure options (LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT)

LPN/LVN License

2-year degree + 2+ years healthcare exp
