Job summary

UnitedHealth Group is seeking a Care Coordinator to manage a panel of members with chronic and complex health needs. You will perform needs assessments, develop person-centered care plans, and collaborate with providers and community resources to remove barriers to care.

The role supports a multi-disciplinary team, with flexible remote work options for Boise, ID area. Strong communication, organization, and care coordination skills are essential.

Qualifications

• Must hold current Idaho license (LCSW/LMSW/LSW/LCPC/LPC/LMFT or LPN/LVN)
• Or have 2-year degree and 2+ years in healthcare
• Experience with Medicaid/Medicare or SDoH needs is required
• Proficiency with MS Office suite is required
• Reliable home workspace and eligible internet access are required
• Ability to travel within the territory up to 50% of time

Responsibilities

• Serve as primary care manager for a panel of members
• Perform comprehensive needs/wellness assessments with focus on medical, behavioral, functional, cultural and social drivers of health
• Collaborate with internal care team and community resources to implement care plans
• Assist members in obtaining HCBS supports and services
• Provide referrals to internal and community resources as appropriate
• Support discharge planning and care transitions after ER visits or facility stays
• Educate and coach members for self-management and healthy lifestyle changes
• Advocate for member needs within the care team
• Assist with nonclinical questions and coordinate with Health Plan and UHC resources
