Job summary

UnitedHealth Group in Idaho is seeking a Care Coordinator to manage a panel of dual-eligible members. You will conduct comprehensive needs assessments, develop individualized care plans, and coordinate with internal teams and community resources to ensure access to services and adherence to care plans.

The role requires clinical and non-clinical activities, with emphasis on person-centered care, coordination, and improving health outcomes.

Qualifications

• Current and unrestricted Idaho license in one of the listed social work or nursing roles.
• 1+ years of experience serving Medicaid/Medicare or SDoH-driven populations.
• Proficiency with MS Office (Word, Excel, Outlook).
• Must have driver’s license and be able to travel up to 50% of the time.

Responsibilities

• Serve as the primary care manager for a panel of dual-eligible members.
• Conduct comprehensive needs assessments and develop person-centered care plans.
• Collaborate with care team and community resources to execute care plans.
• Assist members in obtaining HCBS supports and services.
• Provide education and coaching to support self-management and healthy lifestyle changes.
