Job summary

UnitedHealth Group is hiring a Care Coordinator to manage a panel of members with chronic and complex health needs. You will conduct comprehensive assessments, develop person-centered care plans, and coordinate with providers and community resources to promote quality care.

Remote-friendly role based in Caldwell, ID with up to 50% travel and a focus on SDoH, behavioral health, and medical needs. You will support the broader care team and guide patients toward optimal health outcomes.

Qualifications

• Current Idaho social work license (LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT) or RN license.
• Minimum 1 year experience with Medicaid/Medicare or SDoH needs.
• Proficient with MS Office (Word, Excel, Outlook).
• Must have quiet home workspace with PHI safeguards.
• Residence with reliable internet and ability to travel up to 50%.
• Licensed to travel within territory and maintain privacy standards.

Responsibilities

• Serve as primary care manager for dual eligible members.
• Conduct needs or wellness assessments, medical/behavioral/social drivers.
• Collaborate with care team, providers, and community resources on plans.
• Assist members with HCBS supports and services.
• Provide referrals to internal/external resources as needed.
• Support discharge planning and care transitions after ER/hospital stay.
• Educate and coach member self-management and healthy living.
• Advocate for member needs within the health care team.
• Address nonclinical questions related to credentialing and claims.
