Job summary

UnitedHealth Group is seeking a Care Coordinator to act as primary care manager for a panel of dual-eligible members with chronic and complex health needs.

You will conduct needs assessments, create individualized care plans, and collaborate with care teams to remove barriers to care, including HCBS supports and referrals. Flexible remote work is available for residents of Coeur d'Alene, ID and surrounding areas.

Qualifications

• Current LCSW, LMSW, LSW, LCPC, LPC, LMFT, or LAMFT in Idaho.
• 1+ years of Medicaid/Medicare or SDoH experience.
• 1+ years of MS Office (Word, Excel, Outlook).
• Driver's license and up to 50% travel.
• Access to a quiet home workspace to protect PHI.
• Reliable high-speed internet at home.

Responsibilities

• Serve as the primary care manager for dual-eligible members.
• Perform comprehensive needs/wellness assessments and develop person-centered care plans.
• Collaborate with internal care teams, providers, and community resources to implement plans.
• Assist members with obtaining HCBS supports and referrals.
• Support care transitions after ER visits or SNFs and coordinate care.
• Educate and coach members for self-management and healthy lifestyle.
• Advocate for member needs with the care team.
• Address nonclinical questions for providers and connect to resources.
