Sentara Health is seeking an Epic Population Health IT Analyst to support and optimize Epic Population Health applications, including Healthy Planet and Inpatient Case Management. This remote role collaborates with cross-functional teams to ensure successful testing, integration, upgrades, and ongoing operational support for Sentara's Population Health initiatives.

Qualifications

• Bachelor's degree required or 5 years of relevant experience in lieu of degree.
• Experience with clinical informatics, population health, or care coordination.
• Knowledge of inpatient nursing workflows and healthcare operations.

Responsibilities

• Support, optimize, test, and maintain Epic Population Health applications (Healthy Planet, Inpatient Case Management, EpicCare Link).
• Assist with application integration, system upgrades, testing, and ongoing operational support.
• Collaborate remotely with cross-functional teams to support Population Health growth and strategy.
