Job summary

Sentara Health is seeking an Epic Population Health IT Analyst to support, optimize, test, and maintain Epic Population Health applications such as Healthy Planet and EpicCare Link. This remote role collaborates with cross-functional teams to ensure seamless integration and ongoing operations in Sentara's Population Health strategy.

The ideal candidate has a nursing or health IT background with 3+ years of healthcare informatics or IT analysis experience and familiarity with Epic Population

Qualifications

• Bachelor's degree in nursing, health info mgmt, healthcare administration, information technology, or related field required.
• 3 years healthcare/clinical informatics or IT analyst experience with degree.
• 5 years experience in lieu of degree accepted.
• Experience with clinical case management, care coordination, or population health programs required.
• Knowledge of inpatient nursing workflows and healthcare operations.

Responsibilities

• Provide day-to-day support, optimization, testing, and maintenance of Epic Population Health applications (Healthy Planet, Inpatient Case Management, EpicCare Link).
• Collaborate with cross-functional teams to support integration, system upgrades, testing, and ongoing operational support.
• Assist in testing, workflow analysis, and application optimization within a remote team environment.
• Support Population Health growth and strategy through monitoring and issue resolution.
