Job summary
Sentara Health is seeking an Epic Population Health IT Analyst to support, optimize, test, and maintain Epic Population Health applications including Healthy Planet and Case Management. This remote role partners with cross-functional teams to support integrations, upgrades, testing, and ongoing operations for Sentara’s Population Health growth.
Ideal candidates hold a related bachelor's degree or equivalent experience, with Epic HP/Healthy Planet or IP Clin Doc/Epic Case Management
Qualifications
- Bachelor's degree or equivalent in a related field is required.
- Epic HP/Healthy Planet or IP Clin Doc or Epic Case Management certifications preferred.
- 3 years of healthcare IT or clinical informatics application support experience with degree; 5 years with no degree.
- Experience in clinical case management, care coordination, or population health programs preferred.
- Strong analytical, problem-solving, communication, and customer service skills.
Responsibilities
- Provide day-to-day support, optimization, testing, and maintenance of Epic Population Health applications.
- Support integration, system upgrades, testing, and ongoing operational support for Population Health growth.
- Collaborate with cross-functional teams in a remote environment.
