Job summary

Sentara Health is seeking an Epic Population Health IT Analyst to support, test, and maintain Epic Population Health applications including Healthy Planet and Inpatient Case Management. This remote role partners with cross-functional teams to implement upgrades, verify workflows, and provide ongoing operational support as Sentara grows its Population Health strategy.

Candidates should have a relevant degree or equivalent experience and experience with Epic Population Health tools; strong

Qualifications

• Bachelor's degree in nursing, HIM, healthcare admin, IT, or related field required.
• 5 years of relevant experience may substitute for degree.
• 3 years of healthcare, clinical informatics, application support, or IT analyst experience.
• Experience in clinical case management or population health required.
• Knowledge of inpatient nursing workflows and healthcare operations.
• Familiarity with Epic Healthy Planet and population health tools preferred.
• Strong analytical, problem-solving, communication, and customer service skills.
• Ability to work remotely with cross-functional teams.
