Job summary

Sentara Health is seeking an Epic Healthy Planet Senior IT Specialty Analyst to support and optimize Healthy Planet care management workflows in a fully remote role. You will partner with care managers and clinical leaders to implement standard, scalable solutions that improve patient care across populations.

The ideal candidate has Epic Healthy Planet certification, experience with related Epic modules, and 5–7 years of relevant work.

Qualifications

• Epic Healthy Planet Certification/Accreditation required.
• Experience with Epic modules such as EpicCare Ambulatory, Reporting Workbench, SlicerDicer.
• Familiarity with CMS quality programs, HEDIS measures, and risk adjustment.
• 5–7 years of relevant experience

Responsibilities

• Provide day-to-day support and optimization of Epic Healthy Planet care management workflows and configuration.
• Develop test plans and manage incident coverage and enhancement requests.
• Collaborate with care managers, clinical/operations leadership and informatics teams.
• Support quarterly update process and Epic best practices to benefit patients and providers.
