Job summary

Pacific Health Group is seeking an ECM Program Manager to lead and expand our Enhanced Care Management program across California. You will supervise Lead Care Managers, drive program quality, and ensure high-quality, person-centered services for high-need populations.

You will coordinate with hospitals, providers, and community partners while monitoring budgets and growth initiatives. Strong leadership, analytics, and California travel are essential.

Qualifications

• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing Enhanced Care Management or intensive care management programs.

Responsibilities

• Lead Lead Care Managers across multiple counties and provide coaching.
• Establish benchmarks, metrics, and QA protocols for program quality.
• Oversee member services, ensure in-person visits, and monitor documentation.
• Serve as liaison with hospitals, providers, and community organizations.
