Job summary

Pacific Health Group seeks an ECM Program Manager to lead the Enhanced Care Management program across California. You will supervise Lead Care Managers, oversee quality, member services, cross-functional partnerships, budgeting, and growth initiatives to close health equity gaps.

This role requires leadership, data-driven decision making, and a commitment to person-centered care. Travel within the hiring counties is expected; remote work within California is supported.

Qualifications

• Bachelor’s or Master’s degree in healthcare-related field.
• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing Enhanced Care Management or intensive care management programs.
• Knowledge of Medicaid/Medicare ECM requirements.
• Strong data-driven program evaluation experience.

Responsibilities

• Lead and supervise Lead Care Managers across counties.
• Oversee program quality, member services, and partnerships.
• Develop benchmarks, metrics, and QA protocols.
• Monitor staff performance and use data for improvements.
• Manage budgets, resources, and staffing.
• Pilot initiatives like telehealth to expand access.
