Job summary

Pacific Health Group is seeking an ECM Program Manager to lead the Enhanced Care Management initiative across California, guiding regional teams to deliver person-centered care and improve health outcomes.

You will oversee quality, member services, and partnerships while driving program growth and compliance with regulatory standards. This role requires travel within the hiring counties and strong leadership skills.

Qualifications

• Bachelor’s or Master’s in Healthcare Administration, Public Health, Nursing, or related field.
• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing ECM or intensive care programs.
• Experience with Medicaid/Medicare ECM requirements.
• Strong analytical and program evaluation skills.

Responsibilities

• Lead and supervise Lead Care Managers across multiple counties.
• Oversee program quality, member services, and cross-functional partnerships.
• Budget, staffing, and resource management for the ECM program.
• Collaborate with hospitals, providers, and community organizations.
• Pilot telehealth and other innovations to expand access to care.
• Prepare quarterly performance reports and maintain stakeholder relationships.
