Job summary

Pacific Health Group is seeking an ECM Program Manager to lead the Enhanced Care Management (ECM) program across California, focusing on high-need populations and health equity.

You will supervise Lead Care Managers, oversee program quality, member services, budgeting, and cross-functional partnerships with hospitals, providers, and community organizations to improve outcomes and reduce readmissions.

Qualifications

• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing Enhanced Care Management programs.
• Must reside within the hiring county and travel up to 60–70% within the county.

Responsibilities

• Lead and supervise Lead Care Managers across counties.
• Oversee program quality, member services, and budgeting.
• Develop budgets and allocate resources; adjust staffing as needed.
• Collaborate with hospitals, providers, and community organizations.
• Pilot new initiatives such as telehealth and improve care transitions.
