Job summary

Pacific Health Group is seeking an ECM Program Manager to lead our Enhanced Care Management program across California. You will supervise Lead Care Managers, ensure program quality, oversee member services, and drive cross-functional partnerships to improve health outcomes for high-need populations.

The role requires 5+ years in healthcare management, with 3+ years in care/case management, plus knowledge of Medicaid/Medicare ECM requirements.

Qualifications

• Bachelor’s or Master’s in Healthcare Administration, Public Health, Nursing, or related field.
• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing Enhanced Care Management or intensive care management programs.
• Understanding of care management principles and healthcare delivery systems.
• Experience with Medicaid/Medicare programs and ECM program requirements.
• Strong analytical skills and data-driven program evaluation.

Responsibilities

• Lead Lead Care Managers across multiple counties and supervise teams.
• Oversee program quality, member services, and cross-functional partnerships.
• Develop and manage program budgets and allocate resources.
• Pilot new initiatives such as telehealth to expand access to care.
• Establish benchmarks, metrics, and QA protocols to track performance.
• Coordinate with hospitals, providers, and community organizations.
