Job summary

Pacific Health Group is seeking an ECM Program Manager to lead the Enhanced Care Management program across California. You will supervise Lead Care Managers, manage budgets, drive program quality, and foster cross-functional partnerships with hospitals, providers, and community organizations.

The role emphasizes compassionate, person‑centered care and health equity. Applicants should have 5+ years in healthcare management, including care management, with experience in Medicaid/Medicare ECM

Qualifications

• Bachelor’s or Master’s in Healthcare Administration, Public Health, Nursing, or related field.
• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing Enhanced Care Management or intensive care management programs.
• Strong understanding of care management principles and healthcare delivery systems.

Responsibilities

• Lead and mentor Lead Care Managers across counties.
• Oversee program quality, member services, and budgeting.
• Collaborate with hospitals, providers, and community partners.
• Monitor outcomes and ensure regulatory standards.
