Job summary

Pacific Health Group is seeking an ECM Program Manager to lead the Enhanced Care Management program across California counties. You will supervise Lead Care Managers, guide program development, and ensure high-quality, person-centered care that aligns with the company’s mission to close health equity gaps.

The role requires 5+ years in healthcare management, 3+ years in care/case management, and 2+ years in ECM programs.

Qualifications

• Bachelor’s or Master’s in Healthcare Administration, Public Health, Nursing, or related field.
• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing Enhanced Care Management or intensive care management programs.

Responsibilities

• Provide leadership over the ECM program and supervise Lead Care Managers.
• Oversee program quality, member services, and cross-functional partnerships.
• Develop and manage budgets, resource allocation, and program growth initiatives.
