Pacific Health Group is seeking an ECM Program Manager to lead the Enhanced Care Management program across California counties. You will supervise Lead Care Managers, ensure program quality, and drive growth while collaborating with hospitals, providers, and community partners.

The role requires deep knowledge of ECM, Medicaid/Medicare, and data-driven evaluation, with a strong emphasis on person-centered care and health equity.

Qualifications

• Leadership of care teams across counties.
• Data-driven program evaluation and QA.
• Knowledge of Medicaid/Medicare ECM requirements.

Responsibilities

• Lead Lead Care Managers across counties; provide coaching.
• Set benchmarks, monitor outcomes, QA protocols.
• Oversee member services and in-person visits; improve engagement.
