Job summary

Pacific Health Group is seeking an ECM Program Manager to lead the Enhanced Care Management program across California. The role focuses on directing regional teams, ensuring high-quality, person-centered care, and advancing initiatives to close health equity gaps.

You will supervise Lead Care Managers, drive program development, and monitor performance with a focus on outcomes, compliance, and stakeholder collaboration.

Qualifications

• Bachelor’s or Master’s in healthcare administration, public health, nursing, or related field.
• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing Enhanced Care Management or intensive care management programs.

Responsibilities

• Lead and supervise ECM program teams across counties, ensuring resources and support for success.
• Oversee program quality, member services, and cross-functional partnerships; manage budgeting and growth initiatives.
• Coordinate with hospitals, providers, and community partners to improve care transitions and reduce readmissions.
