Job summary

Pacific Health Group is seeking an ECM Program Manager to lead our Enhanced Care Management program across California. You’ll supervise Lead Care Managers, oversee quality, member services, and cross-functional partnerships to improve outcomes and health equity.

This role requires strong leadership, data-driven oversight, and the ability to travel up to 60-70% within the county to conduct in-person visits, with fully remote work within California supported by a company laptop.

Qualifications

• Bachelor’s or Master’s in Healthcare Administration, Public Health, Nursing, or related field.
• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing Enhanced Care Management or intensive care management programs.
• Understanding of care management principles and healthcare delivery systems.

Responsibilities

• Lead Team Leadership & Supervision across multiple counties.
• Oversee program quality, member services, and regulatory standards.
• Manage budgets and resource allocation for program growth.
• Collaborate with hospitals, providers, and community partners.
• Pilot innovations such as telehealth to expand access to care.
