Pacific Health Group is seeking an ECM Program Manager to lead Enhanced Care Management across multiple counties in California. You will supervise Lead Care Managers and oversee program quality, member services, and cross‑functional partnerships to improve outcomes and advance health equity.

The role emphasizes leadership, data‑driven evaluation, and growth initiatives, including telehealth pilots, while ensuring Medicaid/Medicare ECM requirements are met.

Qualifications

• Bachelor’s or Master’s in Healthcare Administration, Public Health, Nursing, or related field.
• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing Enhanced Care Management or intensive care management programs.
• Knowledge of Medicaid/Medicare ECM requirements.

Responsibilities

• Lead and supervise Lead Care Managers across counties.
• Oversee program quality, member services, and budgets.
• Coordinate with hospitals, providers, and community organizations.
• Drive program growth through innovation and telehealth pilots.
• Monitor outcomes and ensure regulatory compliance.
