Job summary

Early Medical is a fully remote healthcare practice focused on longevity and personalized care. The Patient Care Coordinator will orchestrate a physician’s practice, delivering concierge‑level coordination—from prescriptions and tests to referrals and follow‑ups—while collaborating with Executive Assistants and external partners.

Qualifications

• Bachelor’s degree or higher preferred.
• 5+ years of experience coordinating patient care in a healthcare setting (primary care/internal medicine/concierge medicine) preferred.
• Experience supporting executives, physicians or senior professionals in a fast-paced environment.
• Experience coordinating multiple complex workflows simultaneously with attention to detail.
• Experience coordinating specialty referrals, laboratory testing, prior authorizations, or complex care plans.

Responsibilities

• Create a premium, concierge-level patient experience and support overall care journeys.
• Coordinate scheduling, testing, referrals, and provider availability to minimize friction.
• Maintain impeccable patient charts and accurate documentation across systems.
• Build and manage external relationships with labs, referal sources, and other providers.
• Ensure timely follow-through on orders, referrals, and care plans.
