Job summary

Ascend Medical is seeking a Care Navigator to be the patient support liaison, guiding new members through individualized care plans, triaging, booking telehealth appointments, and coordinating with the Care Coordinator to ensure follow-up.

This role emphasizes compassionate, patient-centered care across psychiatry and primary care, with a flexible schedule, remote-friendly environment, and a culture that rewards initiative and teamwork.

Qualifications

• High school diploma or equivalent is required.
• Graduate from a nationally accredited Medical Assistant program OR equivalent healthcare preparation is required.
• Certified Clinical Medical Assistant (CCMA) preferred.
• Minimum of 2 years MA experience in a healthcare setting is required.
• Must be current in CPR certification.
• Experience using Athena EMR is preferred.

Responsibilities

• Book telehealth appointments, triage patients, and record patient interview, history & chief complaint using Ascend's EMR.
• Facilitate telehealth consultations with chart review and patient orientation to technology.
• Prepare patients for examinations and assist with referrals as needed.
• Coordinate with Care Coordinator to ensure care plans and follow-up appointments.
• Organize inbound/outbound communications (calls, chats, emails).
• Educate members on Ascend services and help navigate health insurance benefits.
