Qualifications

• High School Diploma or GED required.

Responsibilities

• Reach out to members telephonically to assist with referrals, authorizations, scheduling, and follow ups.
• Create cases, tasks, and complete documentation in the case management module.
• Coordinate services and navigate care plans with the interdisciplinary team.
• Assist with outreach activities and maintain member records.
• Maintain confidentiality and meet deadlines per department policies.
