Tuesday Health is a value-based palliative care provider focused on transforming serious illness and end-of-life care. We deliver goal-centered care to alleviate symptoms and emotional stress for patients and their caregivers, wherever they call home.

The Complex Care Navigator RN will lead, coordinate, and deliver person-centered care within a multidisciplinary team, using Tuesday Health’s electronic systems to ensure a streamlined, technology-enabled approach.

Qualifications

• Active and unrestricted RN license in Ohio with no board actions.
• Experience in clinical/medical settings preferred.
• Experience in multidisciplinary care models across providers, health plan care management, and clinicians.
• Strong communication and organizational skills.

Responsabilités

• Administer multiple assessments and screening tools within the clinical model.
• Interpret responses to assessments and prioritize needs based on results.
• Be accountable for care plan development within the multidisciplinary care team.
• Lead internal and joint rounds to optimize the member experience.
• Learn the Tuesday Health electronic systems and tools to deliver coordinated care.
• Build trusting relationships with members through listening and empathy.
• Serve as the quarterback of the multidisciplinary team transforming care for members.
