Ideal candidates will hold a MA RN license, have clinical experience, and be comfortable using electronic systems. The position supports remote work with occasional onsite sessions in Massachusetts and requires driving as needed.

Qualifications

• Active and unrestricted RN license in MA.
• Experience in clinical settings and multidisciplinary care models.
• Ability to navigate documentation systems and learn new tools quickly.
• Valid driver's license and automobile insurance.

Responsibilities

• Administer multiple assessments and screening tools throughout the care timeline.
• Interpret responses and prioritize needs based on assessment results.
• Be accountable for care plan development within the care team.
• Lead in internal and joint rounds to optimize the member experience.
• Learn and use Tuesday Health’s electronic systems and tools for coordinated care.
• Build strong, trusting relationships with members through empathy.
• Act as quarterback of the multidisciplinary team to transform care.
