The Coordinating Center in Maryland seeks a full-time Coordinator of Community Services to manage a caseload of individuals with intellectual/developmental disabilities, helping them access medical and community supports through person-centered planning. This teleworking role requires quarterly client visits in Montgomery County, MD and a valid driver’s license.

Qualifications

• Bachelor’s degree in a human service focus, or an Associate’s degree plus two years of human service experience, or seven years of related work.
• Experience coordinating services for Medicaid/waiver programs or for individuals with ID/DD is preferred.
• Competency in negotiation, advocacy, relationship-building, person- and family-centered care, and community resources.
• Access to a reliable automobile and a valid driver’s license.
• Excellent written and spoken communication; proficiency with Word, Excel, and PowerPoint.
• Ability to manage complex, multi-task workflows.

Responsabilités

• Maintain a caseload of 30–40 clients with intellectual/developmental disabilities.
• Advocate for clients and support self-advocacy in service planning.
• Develop appropriate Person-Centered Plans per COMAR regulations and agency policies.
• Research and implement services to meet client needs in a timely manner.
• Monitor service provision and gather information relevant to supports.
• Visit clients in settings where supports are provided, at least four times per plan year.
• Assist with enrollment activities to ensure continuity of supports.
• Coordinate PCP and interdisciplinary meetings and annual reviews.
• Conduct required assessments per DDA regulations.
• Promote participation in team processes and resolve conflicts.
• Inform participants of provider choices annually.
• Respond to incident reports to ensure safety of clients.
• Document services to meet billing standards within LTSS and internal systems.
• Attend required meetings and trainings.
• Maintain professional relationships with external providers.
