Job summary

Service Coordination, Inc. is hiring Service Coordinators to provide case management within the community in Maryland. The role emphasizes advocacy, resource coordination, and facilitating person-centered planning for individuals with developmental and intellectual disabilities.

Salary ranges between $64,395 and $79,861 with education and experience-based placement, plus travel reimbursement, quarterly bonuses, and extensive benefits. Training and equipment are provided to support success.

Qualifications

• A bachelor's degree from an accredited education program in a human services field; OR
• A bachelor's degree in a non-human services field, from an accredited education program, with two years of experience in a human services field

Responsibilities

• Advocate on behalf of individuals, provide resources, and educate them to become more self-determined.
• Collect information for eligibility for DDA services and recommend eligibility and priority to DDA.
• Plan for, facilitate, develop, and continually update Person Centered Plans (PCPs).
• Research, identify, refer, and coordinate resources and services to achieve PCP outcomes.
• Monitor Person Centered Plans (PCP) and conduct follow-up activities.
• Adhere to SCI policies and confidentiality, HIPAA, and related procedures.
