Able Health Services Inc. seeks a Coordinator of Community Services to join our hybrid team in Maryland. This role provides targeted case management, advocacy, and resource coordination to support individuals.

You will help develop and monitor Person-Centered Plans, plan and attend team meetings, and ensure services remain aligned with goals. You will also conduct home and community visits, support transitions, document activities for billing, and respond to incident reports to safeguard the

• Bachelor’s degree in a human services field or an equivalent combination
• Associates degree with 2 years experience in human services, or 7 years in the human service field
• Proven advocacy skills and ability to coordinate resources and services

Responsabilidades

• Provide Targeted Case Management and advocacy to individuals
• Develop and monitor Person-Centered Plans
• Visit individuals at least three times per PCP year to ensure continuity of supports
• Assist with transitions and transfer activities
• Document activities for billing and reporting
• Respond to incident reports promptly and maintain health and safety standards
• Explore alternative supports and funding sources as needed

Human services experience

Educación

Bachelor's degree in human services
