You will collaborate with a multidisciplinary team, document progress, and ensure timely follow-ups. A Bachelor’s degree or RN license is required, with reliable transportation and internet access for remote tasks.

Qualifications

• Bachelor’s degree or RN license is required.
• Reliable transportation with valid driver's license and insurance.
• Must have internet access for telecommuting.
• Must live in service area to be considered.

Responsabilités

• Manage a caseload of individuals with diverse needs to promote independence.
• Develop community links and coordinate resources to meet plan goals.
• Monitor and assess service quality; report findings and act on them.
• Document activities within three business days of action; maintain monthly billable hours.
• Coordinate ISP team activities and advocate for client rights.
