Job summary

Service Coordination, Inc. is seeking a Service Coordinator to provide case management for individuals with intellectual and developmental disabilities in the Oakland, MD area.

You will establish and monitor Person-Centered Plans, work with families and providers, and ensure services meet state requirements and SCI standards. The role involves travel to community settings, training and ongoing professional development, and a focus on empowering clients through self-determination and coordinated

Qualifications

• Bachelor's degree from an accredited education program in a human services field.
• Bachelor's degree in a non-human services field with two years of human services experience.

Responsibilities

• Provide Case Management services to individuals with intellectual and/or developmental disabilities.
• Establish and monitor a Person-Centered Plan (PCP) and annual goals.
• Coordinate with individuals, families and external providers to secure ongoing services.
• Ensure services meet state-mandated standards and SCI policies.

Education

Bachelor's degree in human services
