Job summary

HOPE Family Health is seeking a Community Health Worker to provide medical and behavioral health telephone support in Westmoreland, TN. The role focuses on follow-up, coordination of services, language access, and referrals to community resources.

Duties include documenting contacts, supporting care plans, and ensuring timely follow-up for patients. The CHW will work with care teams to reduce barriers to care, communicate effectively with patients, and participate in team trainings and quality

Qualifications

• High school diploma or equivalent required.
• Ability to communicate clearly with patients and care teams.
• Experience with electronic health records or standard office software preferred.
• Additional language proficiency or interpreter qualifications may be required for specific assignments.

Responsibilities

• Build rapport with patients and families and communicate in a culturally responsive manner.
• Contact patients by phone or other approved methods; relay information and coordinate care with licensed staff.
• Help patients understand appointments, care plans, referrals, and next steps without providing clinical advice.
• Identify barriers to care and connect patients with HOPE programs and resources.
• Coordinate with medical and behavioral health teams to support continuity of care.
