Responsibilities

• Identify barriers to care and connect patients with HOPE programs and resources.
• Coordinate with medical, behavioral health, pharmacy, and external partners for continuity of care.
• Maintain accurate records in the electronic health record and protect confidentiality.
• Participate in team meetings and quality improvement activities.
