HOPE Family Health Services in Westmoreland, TN, is seeking a Patient Navigator – Telephone to support behavioral health care coordination. You will triage calls, relay clinical information, and document interactions in the EHR to ensure timely, patient-centered care.

The role involves liaising with the behavioral health team and patients to facilitate care access. The ideal candidate will have MA or LPN certification, a current BLS, and strong communication skills, with experience in healthcare

Qualifications

• High School Diploma or equivalent.
• Medical Assistant or Licensed Practical Nurse certification.
• Current BLS certification.
• Proficient in basic computer use and EHR documentation.
• Strong problem-solving skills and ability to exercise sound judgment.

Responsabilités

• Triages incoming calls and assesses urgency using clinical judgment.
• Communicates with patients about care needs, results, and provider-directed information clearly.
• Documents all patient interactions in the EHR in real time.
• Provides basic patient education and counseling, including lab and behavioral health prep.
• Interprets for patients when qualified to facilitate communication.
