• Help patients understand appointments, care plans, referrals, tests, and next steps with care team-approved information.
• Identify barriers to care (transportation, insurance, cost, language, housing) and connect to HOPE programs and resources.
• Coordinate with medical and behavioral health teams and external partners to support continuity of care.
• Maintain timely, accurate records of contacts, referrals, and follow-up in electronic health records.
• Participate in team meetings, training, and quality improvement activities.
