Job summary

You will assess medical, behavioral and social needs, develop person-centered care plans, and coordinate with providers and community resources to improve outcomes.

Qualifications

• Must meet one: social work licensure or 4-year behavioral health degree + 3+ years' experience or LPN with 5+ years.
• At least 2 years in community health or healthcare settings.
• 1+ year working with local providers/organizations addressing SDoH.
• 1+ year MS Office (Word, Excel, Outlook).
• Colorado resident with driver's license and reliable transportation.

Responsibilities

• Engage members and families to complete comprehensive needs assessments.
• Develop and implement individualized, person-centered care plans.
• Collaborate with internal care team, providers and community resources to implement care plans.
• Provide referrals and linkage to internal or community services as needed.
• Coordinate with community partners to support members with complex needs.
