Job summary

UnitedHealth Group is seeking a Care Coordinator to manage a panel of members with chronic and complex health care needs in Caldwell, ID. You will support medical, behavioral, and social drivers of health through person-centered care plans and collaboration with internal teams.

This fast-paced role requires multitasking, organization, and strong communication to improve care quality. Remote work flexibility is available; a valid Idaho license and experience with Medicaid/Medicare are preferred.

Qualifications

• Current Idaho license in LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT or LPN/LVN.
• 1+ years of experience with Medicaid/Medicare or social drivers of health.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Access to a quiet home workspace with ability to protect PHI.
• Willingness to travel within assigned territory up to 50% of the time.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs/wellness assessments across medical, behavioral, functional, cultural, and social drivers of health.
• Develop individualized, person-centered care plans with clear goals and interventions.
• Collaborate with care team, providers, and community resources to implement plans.
• Assist members with HCBS supports and services.
• Support discharge planning and care transitions after ER visits or inpatient stays.
• Educate and coach members for self-management of health and lifestyle changes.
• Advocate for member needs within the health care team.
• Coordinate non-clinical inquiries with credentialing, claims, or health plan resources.
