Qualifications

• Must meet licensure requirements listed for Idaho.
• 1+ years of Medicaid/Medicare or SDoH experience.
• 1+ years using MS Office (Word/Excel/Outlook).
• Access to a quiet home workspace that protects PHI.
• Ability to travel within territory up to 50% with reliable transport.
• Must have a usable high-speed internet connection.

Responsibilities

• Serve as primary care manager for dual eligible members.
• Conduct comprehensive needs assessments including medical, behavioral, social drivers of health.
• Develop and implement person-centered care plans with goals and interventions.
• Collaborate with internal team and community resources to remove barriers.
• Support care transitions and discharge planning as needed.
• Provide education and coaching for member self-management.
• Advocate for member needs within the health care team.
